Facility Evaluation Report

Licensing Program Analyst (LPA) Melissa Parks arrived unannounced on Tuesday October 28, 2025 to continue and complete the annual inspection.

LPA reviewed staff files. LPA and Rouzbeh toured the facility together. LPA observed resident bedrooms, bathrooms, laundry room, garage, kitchen, and backyard. LPA observed the dining room to be converted to a staff bedroom, with a curtain as privacy. Water temperatures were within the required range. LPA observed fire extinguishers with current inspection tagw. LPA observed carbon monoxide detectors. Facility had a fully stocked first aid kit.

LPA obtained a copy of the current liability insurance and LIC500.

Exit interview conducted. A copy of this report and appeal rights were provided to the facility.
