Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday December 23, 2025 to obtain additional information regarding R1. LPA spoke with Kristina Ivascu, Administrator, on the phone regarding the visit.

LPA spoke with R1. LPA obtained copies of R1's physicians report, LIC625, LIC601 and care plan.

No deficiencies cited. A copy of this report was provided to the Administrator.
