Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct an annual inspection. LPA met with Director Orlando Perez during today's inspection. LPA ensured they applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: Masks.

LPA toured facility with Director to ensure health and safety of clients in care. LPA toured common areas, kitchen area, and bathrooms. In the areas toured no immediate health, safety, or personal rights violations were observed. Director stated there has been no positive COVID cases at the facility, and sufficient amount of PPE available for staff. There is a locked storage for medications and toxins. LPA reviewed the infection control domain guidance with Director.

No deficiencies are being cited as a result of todays inspection.
