Facility Evaluation Report
On August 5, 2026, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding an incident report that was submitted to the Department. LPA met with Supervisor and explained the purpose of the visit.
LPA was informed there are five staff present working at the day program, along with two other staff with participants on an outing.
The incident report stated participant (P1) returned home from day program which then family member observed P1's garment to be loose. Due to P1's limited cognitive abilities, it was undetermined how the garment became loose and if it was intentional or accidental. Additionally, it was undetermined who was the individual who loosen P1's garment.
LPA and Supervisor discussed that nothing unusual was observed by staff as day program enforce participants to keep hands and feet to themselves. Day program will continue to monitor and document participants' behavior.
Exit interview conducted and a copy of the report was provided.
