The inspector’s account

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct an annual inspection and met with Michael Chenko, Supervisor, and Jana Binevskiy, Program Coordinator, stating the reason for the inspection. LPA observed (7) staff members present. The Administrator, Tanya Chenko, arrived at 11:00 am. The location is licensed for an adult day program for (30) clients, all of whom must be ambulatory. LPA observed all clients to be ambulatory. There were (24) clients present. At 11:00 am, approximately (10) clients attended an off-site lunch outing and were driven by staff in (2) facility vans.

LPA, the Program Coordinator and staff supervisor toured the interior/exterior of the facility, including the common activity rooms, quiet room, kitchen area, and bathrooms. LPA observed clients to be participating in a variety of activities in the front area. There were no immediate health, safety, or personal rights violations observed. The facility was clean, in good repair and free from odors. There is a locked area for toxins in the kitchen and tables/chairs for clients to eat their lunch/snacks. There is a kitchen area with a water cooler. Backpacks and jackets are organized.Hot water measured 122*F in the kitchen- Administrator was not able to adjust the temperature and will contact maintenance staff who recently installed the new water heater. The fire extinguishers were last serviced 6/30/24, and upgraded fire/smoke alarms were recently installed. Administrator to submit documents for an increase in capacity to (45) clients. Emergency exits are clear/accessible. The First Aid kit is complete. Program Rules and Personal Rights are posted along with volunteer schedule and activity calendar. Facility temperature measured 71*F. (1) client takes medications while attending program. Medications are locked in the office. Disaster drills are done monthly.

LPA reviewed (5) client files and (5) staff files. All files contained were complete and had current paperwork. Staff training is completed monthly with varying topics. All staff is cleared/associated and have current First Aid/CPR certifications. There were no deficiencies observed. Exit interview conducted.
