Facility Evaluation Report

Licensing Program Analyst (LPA) Bethany Mirlohi arrived unannounced to conduct a case management visit. LPA met with Administrator Henry Cole during today's visit.

LPA received an incident report from the facility concerning a medication error that was observed on 5/20/23. On January 31, 2023, facility Medication technician accidentally inputted a discontinue order for R1's Donepezil medication. R1 did not receive Donepezil medication from February 1, 2023 through May 20, 2023. Medication error was reported to R1's physician, and responsible party were notified. Medication was reordered and is now being given to resident as ordered. Administrator stated training was provided to staff as a result of the error.
