On 10/25/23, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with licensee .
LPA and licensee discussed incident reports submitted regarding falls by R1, R2 and R3.
Care plans details, policies and training on policies/ procedures for determining wen to call PCP or emergency responders.
Current plans were discussed regarding R2 and R3.
The home is clean and resident care is provided for resident needs at this time.
As a result of today’s inspection, no deficiencies were noted.
