Licensing Program Analysts (LPAs) Melissa Parks and Marisa Chiarelli arrived on Monday November 24, 2025 to conduct a case management visit regarding past due annual fee.
LPAs met with Administrator Silvia and explained the purpose of visit. LPAs provided Silvia with the PIN number and balance due. LPA spoke with the Licensee on the phone regarding the past due balance. LPAs instructed that if the balance was not paid by December 1, 2025, an additional case management will be required.
Exit interview conducted. A copy of this report was provided to the facility.
