Facility Evaluation Report
Licensing Program Analyst (LPA) Graham Gunby arrived unannounced to conduct an annual inspection. LPA met with Regional Director, Nati Mauga, during today's inspection.
LPA toured facility with Regional Director to ensure health and safety of clients in care. LPA toured common areas, kitchen area, and bathrooms. Facility provides snacks throughout the day. Kitchen was clean and in good repair. In the areas toured no immediate health, safety, or personal rights violations were observed. There is locked storage for all toxins and sharps. Fire extinguishers are located in the common areas and were last serviced on 09/10/2025.
LPA reviewed 3 client files and 3 staff files. All clients records contained the required documentation. A review of staff records indicates that all facility staff have received criminal record clearances and/or are associated to this facility. Staff records reviewed indicated current first aid certificates and training completed.
No deficiencies are being cited as a result of todays inspection.
Exit interview conducted and copy of the report was emailed to the Regional Director.
