Facility Evaluation Report

LPA Hiratsuka conducted this unannounced annual visit. LPA arrived prior to the program starting and was present when clients started arriving.

This facility has a fire clearance for 45 non-ambulatory clients. The increase in capacity occurred on August 20, 2024. This facility has a large front common room and a large common room in the back. There are two small classrooms. The back room is the main common area that has a sink and small cooking appliances for cooking classes. There are three restrooms.
