Licensing Program Analyst (LPA) Melissa Parks arrived on Wednesday August 5, 2026, to conduct an unannounced case management visit. LPA met with Administrator Roxana.
LPA and Roxana discussed recent improvements at the facility. LPA toured the facility. LPA requested the following documents from Roxana by the end of the week: LIC501, LIC503 and TB, administrator certificate, LIC200, first aid/cpr, and LIC500.
No deficiencies cited. Exit interview conducted. A copy of this report was provided to the facility.
