Facility Evaluation Report
On 08/28/2025, Licensing Program Analyst (LPA) Graham Gunby arrived to the facility unannounced to conduct a Required 1- year annual inspection. LPA met with Administrator, Petre Mile, to conduct a tour and file review.
LPA reviewed (2) resident files and (1) staff file. All files contained the correct documentation.
During today's inspection, LPA and Administrator conducted a tour of the exterior and interior of facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents bedrooms, bathroom, medication closet, kitchen, backyard and the common areas.
LPA observed toxins, medication and sharps to be locked and secured. Fire extinguisher is located in the kitchen. LPA observed complete first-aid kit. LPA completed the CARE tool and found the facility to be in compliance. Facility meets the perishable and nonperishable food requirements. In the areas toured, there were no health or safety violations observed.
No deficiencies are being cited during todays visit. Exit interview conducted and a copy of the report was emailed.
