Licensing Program Analysts (LPAs) Melissa Parks and Bethany Mirlohi arrived on Saturday May 2, 2026 to conduct a case management visit regarding the Temporary Suspension Order which was served on May 1, 2026 and will be in effect on Monday May 4, 2026 at 5:00 pm.

LPAs conducted a health and safety tour of the facility. At this time, there are 5 remaining residents. No deficiencies cited. A copy of this report was provided to the facility.
