Facility Evaluation Report
Licensing Program Analyst (LPA) Melissa Parks arrived on Thursday January 8, 2026, to conduct the unannounced annual inspection.
During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (5) and staff (2) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training.
LPA and Licensee Lisa toured the facility together to ensure the health and safety of residents in care. The areas toured included resident bedrooms, bathrooms, kitchen, common area, and backyard. Facility had current inspection tag on fire extinguishers. Water temperatures were within the required range. Facility had a fully stocked first aid kit. All required postings were observed.
LPA obtained a copy of the current liability insurance.
Exit interview conducted. No deficiencies cited. A copy of this report was provided to the facility.
