Facility Evaluation Report

On 08/26/2025, Licensing Program Analyst (LPA) Graham Gunby arrived to the facility unannounced to conduct a Required 1- year annual inspection. LPA met with Administrator, Raisa Oselsky, to conduct the inspection.

The facility is licensed for (6) hospice residents. LPA was informed there are (1) residents on hospice services.

During today's inspection, LPA and Administrator conducted a tour of the exterior and interior of facility to ensure the health and safety of residents in care. Areas toured included but not limited to: residents bedrooms, bathroom, medication room, kitchen, backyard and the common areas.

LPA observed toxins, medication and sharps to be locked and secured. LPA completed the CARE tool and found the facility to be in compliance.

LPA observed a baby monitor in R2's room. There is no documentation from the responsible persons, and R2 requested the camera is removed.

Exit interview conducted and a copy of the report was emailed.
