Facility Evaluation Report

On 04/04/25 at 1:30 pm, a virtual office meeting was held with Sacramento North Regional Office using Microsoft Teams.

Present in the meeting was: Licensee, Daniel Popescu, Licensing Program Manager (LPM) Troy Ordonez, and Licensing Program Analyst (LPA) Graham Gunby.

The purpose of today's meeting was to discuss a pending change in the facility's ownership. Specifically discussed were the next steps needed with the Department to initiate a change in ownership for a new facility license.

Also discussed is the facility's requirement to issue a notice to all residents and their responsible person(s) advising of the pending change in ownership, and how a copy of that notice needs to be submitted to the Department within (5) days of its issuance.

LPM emphasized how communication needs to be ongoing with the Department during the ownership transition, and for any other significant changes, such as a change to the property ownership, and the facility's first point of contact is the assigned LPA.

A copy of this report was emailed to Daniel Popescu, Licensee, following today's meeting. A signed copy will be returned to the LPA by COB 04/07/25. There are no deficiencies issued in this report.
