Facility Evaluation Report

LPA Lusby arrived on Friday October 21, 2021 to conduct a case management visit. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask.

The purpose of this visit was to verify that S1 was no longer employed and no longer associated to the facility. Administrator Sabrina confirmed S1's termination on 10/19/2020 and that S1 has not been in the facility since.

Exit interview conducted and a copy of the report was given.
