The inspector’s account

Licensing Program Analyst (LPA) Melissa Parks arrived on Wednesday November 12, 2025, to conduct a case management due to an incident that occurred on 10/28/2025 and subsequently reported to the Department. LPA met with Sheryl and explained the purpose of the visit.

LPA learned that on October 28, 2025, R1 was upset and sent a series to texts to staff due to not having enough of staff's attention. R1 later left the facility. Staff found R1 at a local park. R1 willingly went back to the facility with staff and apologized. Later, R1 became upset with another resident at the facility. R1's behavior escalated to shouting and pacing. Staff attempted to redirect but were unsuccessful. Later that night, R1 texted pictures of their wrist which was cut to the Administrator, which showed that R1 was attempted self-harm. The facility called 911 to transport R1 to the hospital. R1 returned to the facility the next day.

Per R1's physicians report, they can leave the facility unassisted. R1 now has weekly psychiatry appointments. The facility has increased safety checks for R1. There was no medication adjustments made at the hospital. The facility will now keep all shaving razors and nail trimming devices locked.

LPA obtained a copy of R1's physicians report and current IPP.
