On 08/21/25 Licensing Program Manager (LPM) Troy Ordonez, Licensing Program Analyst (LPA) Graham Gunby and LPA Cassandra Mikkelson arrived unannounced at the facility to investigate a complaint. LPA met with Administrator, Yasser Patawaren, to explain the purpose of the visit. During this visit LPM and LPAs observed a broken window to a residents bedroom. The window appeared to be taped on the edged, but when removing the blinds, the window fell to the ground. Administrator removed broken window and LPAs did not observe any broken glass in the room.
