Complaint Investigation Report

During the complaint investigation, LPA reviewed training records for staff. In addition, interviews were conducted. Training records for kitchen staff show staff met sufficient training hours in shadowing and in-class training. This agency has investigated the complaint alleging (Staff are not trained). We have found the complaint was UNFOUNDED, meaning that the allegation is false, could not have happened, and/or is without a reasonable basis.

Exit interview was conducted with Administrator and a copy of this report was provided to the facility. The signature of Administrator on these forms acknowledges receipt of these documents.
