Facility Evaluation Report

LPA Parks arrived on Friday November 4, 2022 to conduct a case management visit. Prior to the visit, LPA completed required COVID-19 testing protocols, and a daily self-screening questionnaire for symptoms of COVID-19 infection to affirm no COVID-19 related symptoms; LPA ensured she applied hand sanitizer before entering the facility and the following Personal Protective Equipment (PPE) was worn: N-95 mask.

LPA was screened prior to entering the facility. LPA toured the facility with Assistant Administrator Judee Castro. LPA and Judee discussed the current staffing at the facility. No health and safety concerns for the clients in care.

LPA requested Espie submit a plan to the Department by end of day November 11, 2022, detailing completion dates for compliance in regards to required paperwork and training.

No deficiencies are being issued at todays visit. Exit interview conducted. A copy of this report was left at the facility.
