Facility Evaluation Report
On May 1,2025, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit met with Caregivers.
This was a follow-up visit for the visit conducted on 4/15/25.
On 4/2/25,it was reported that R1 had an unwitnessed fall in their room. R1's representative arrived shortly after the fall. 9-1-1 was called. R1 sustained fractures and underwent surgery for injuries before returning to the facility.
At today's visit, LPA interviewed S1 and S2 regarding the 4/2/25 incident with R1. Statements found that R1's fall occurred with a staff present and R1 moved unexpectedly during incontinence care.
At today's visit, R1 was observed to be sleeping. LPA was informed that PT has begun for R1.
LPA finds that R1 was receiving needed care at the time of the 4/2/25 fall. LPA discussed and advised a number of considerations for R1's care plan in order to reduce the likelihood of a similar incident.
As a result of today’s inspection, no deficiencies were noted at this time
