Facility Evaluation Report
On April 15, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while delivering complaint findings and met with Caregivers.
On 4/2/25, R1 had an unwitnessed fall in their room. R1's representative arrived shortly after the fall. 9-1-1 was called. R1 sustained fractures and underwent surgery for injuries before returning to the facility.
At today's visit, LPA observed R1 who was sleeping. R1 is receiving home health services.
LPA interviewed a resident in a room adjacent to R1's.
LPA requested the phone numbers of the staff (S1 and S2) who were working at the time of R1's 4/2/25 fall. Numbers requested by 4/17/25.
As a result of today’s inspection, no deficiencies were noted at this time
