Facility Evaluation Report
On 08/11/25, Licensing Program Analyst (LPA) Talwinder Bains arrived to conduct a case management visit to facility. LPA met with Staff, Brandy Baggett (BB) and explained the purpose of the visit.
BB stated that they were overseeing the operations at this facility as current administrator is in-active for now.
Department requested that facility should hire a qualified administrator for this facility per Title 22 Regulations by 09/10/25 and notify the Department once completed. In the meantime, facility will send updated LIC308 to Department by 08/18/25 to ensure that there is designee administrator who will ensure the smooth operations at this facility.
As a result of today’s visit, no deficiencies were observed or cited.
Exit interview was conducted and a copy was left at the facility.
