Licensing Program Analyst (LPA) Cassandra Mikkelson arrived at the facility to conduct the annual inspection utilizing the inspection tool.

There are currently no clients residing in the home. All clients were relocated on September 25,2025.

No staff were present at the facility when LPA arrived so LPA could not enter the facility.
