The inspector’s account

LPA Hiratsuka, conducted this unannounced annual visit. LPA toured the facility with Donald Wallace. Gina Wallace showed up during the visit. This facility is split into two sections; living quarters for the licensee which is on the left when facing the facility and the living quarters for the residents, which is on the right when facing the facility. The living quarters for the residents consist of three floors; first floor has two full common bathrooms that have a couple of steps to enter them and four shared bedrooms and two private. The second floor; which has an entrance from the outside staircase or from the staff living quarters, has one full common bathroom, two shared resident rooms, a dining and living room. The third floor has one shared resident room, one full bathroom, and one storage room. The third floor can only be entered from the second floor by a steep stairway or the staff living quarters. The kitchen for the facility is on the staff living quarters side. It is separated from the resident living side by a door and a window. There is a snack area for the residents. There is a deck on the backside of the facility on the second floor that each facility room has a door leading to it. The main floor has an entrance to the outside. The staff living quarters is off-limits to the residents. Staff and resident records were reviewed. Several issues were discussed during this visit.
