The inspector’s account

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived at the facility and met with Levi Magurean. LPA discussed the reason for the visit.

LPA observed one (1) staff working at the facility who is not fingerprint cleared. Administrator stated they have been working since February 1, 2025 and staff were not fingerprint cleared. Fingerprints were submitted to DOJ but LIC9163 was missing information so fingerprints could not be processed. LPA and Administrator discussed that all staff must be fingerprint cleared prior to working at the facility.

As a result of this visit, a deficiency has been cited on the 809-D. A civil penalty has been assessed. Copy of report and appeal rights provided to Administrator.
