Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Administrator Gary Johnson.
LPA is following up on an incident report received on 11/29/2025. Staff S1 and Client C1 got into a physical altercation at the home resulting in S1 punching C1 in the face. C1 sustained injuries to their left cheekbone/facial area.
Facility followed proper protocols, removing S1 from the home immediately and getting C1 care for injuries.
At this time, no citations are being issued.
