Facility Evaluation Report

On 3/26/2025 LPA Tryon returned to the home to complete the annual visit started on 1/8/2025.

As I drove up to the house staff Steven Mackie was just pulling out to pick up a resident. He said there was staff in the house. I let him know I am just finishing the paperwork for the annual started on 1/8/25; will check out the new floor, finish up and it will just be a quick visit. I asked if the home has taken care of the issues that had been found by Alta Ca RC earlier; he said as far as he knows it is taken care of.

LPA went inside and spoke with staff Gray Johnson.

Residents were all in their rooms relaxing.

The home has new laminate flooring throughout most of the main floor, including the living room area, dining area, office and bedrooms. The floor looks nice.

At this time the CARE Tool has been completed.

No issues noted at this visit. No deficiencies were cited.
