Facility Evaluation Report

On 8-16-25 at 6:30pm Licensing Program Analyst (LPA) Sarah Benson arrived unannounced at the facility to conduct a health and safety case management visit. LPA met with Administrator Paunitta Gray and explained the purpose of the visit.

LPA and Administrator inspected the facility to check the health and safety of residents in care. Areas toured included but not limited to five (5) residents rooms, three (3) bathrooms, kitchen, common areas and the yard. LPA observed that the facility was clean and odor free. LPA Benson interviewed the resident and observed the residents medications. LPA observed the resident to be very independent.

LPA Benson observed no immediate health and safety violations.
