Facility Evaluation Report

An office meeting was held on May 18, 2026 at 11:00 am. The purpose of the meeting was to review the Stipulation and Waiver; and Order adopted by the Department on April 28, 2026.

The following were in attendance: Regional Manager (RM) Harpreet Humpal, Licensing Program Manager (LPM) Laura Munoz, and Licensing Program Analyst (LPA) Cassandra Mikkelson and Respondent Paunitta Gray.

LPM Munoz discussed the purpose of the meeting and read the Stipulation and Waiver, and Order in its entirety with all in attendance, including, but not limited, as follows:

 Revocation of License and Administrator Certificate

 Waiver of Application; Conditions of Employment

 Waiver of Hearing Rights, Appeal/Modification Rights, and Claims

The Respondent indicated they do not have any questions and expressed their understanding. LPM Munoz stated that a report would be drafted documenting today's discussion.

Per the California Code of Regulations, Title 22, Division 6, Chapter 8, no violations are cited in this report. An exit interview was conducted and signature on this report acknowledges the receipt.
