Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a health and safety case management visit. LPA met with Administrator and explained the purpose of the visit.
Today's visit, LPA gathered additional information regarding R1's primary care physician. Additionally, LPA and Administrator conducted a tour of the facility to ensure the health and safety of residents in care. No immediate health, safety or personal rights violation observed.
Exit interview conducted and copy of report was provided.
