Facility Evaluation Report
LPA Tryon arrived at the facility on 11/18/2021 to perform an annual visit using the Infection Control Domain. Prior to the visit, LPA had checked with the facility to ensure they do not have any COVID Positive Residents or staff. LPA did a self-screening by taking temperature and reviewing symptoms. LPA wore a surgical mask and used hand sanitizer. The facility is doing screening of visitors at the front entrance including temperature check, screening of symptoms, requesting use of hand sanitizer, etc.
LPA met with Program Director/Administrator Brandon Wautlet..
LPA toured the facility including common areas, dining room, resident bedrooms, bathrooms, hallways, , yard. There appears to be a good supply of PPE, soap, paper towels, cleaners, etc. There is a good supply of perishable, frozen and canned/dry goods food. The facility appears to be clean and generally in good repair.
LPA reviewed the infection control domain with the Administrator. Mitigation Plan is in place.
The facility appears to be in substantial compliance at this time.
