Facility Evaluation Report
Licensing Program Analyst (LPA) Hiratsuka, conducted this unannounced annual inspection. LPA toured with Caregiver Aida Bolacito. Jerry Andrada showed up and completed visit with LPA. This facility has three buildings on the property. The first building is the main building. The main building has four resident rooms and one staff room The staff and one resident room in the main building has stairs leading to the rooms in the back of the building on the same side as the garage. There are four full bathrooms in this building. There is a kitchen, dining, and common area. The stove is covered by a cover to keep the knobs inaccessible to the residents. The medications are stored in the dining area in a locked closet at the top of a short staircase. There is an ample supply of food. There is a second building in the back that has four resident rooms and a large common area. The four resident rooms all have full bathrooms. There is a covered walkway that connects the two buildings. The laundry is in a separate building that is attached to a caregiver's room. There is a detached garage in the front. The laundry room is locked and not accessible to residents. When residents are in the back building, there is a staff person there. All exits from the facility property have motion sensors on them. Staff and resident records were reviewed. Several topics were discussed.
