Facility Evaluation Report
Licensing Program Analyst (LPA) Cassandra Mikkelson visited the program to conduct a required annual review. LPA met with Johnna Evans. The program had 32 participants at program on this date.
LPA toured the program site including but not limited to classrooms, kitchen, bathrooms, and offices. The program appears to be clean, building and furnishings are in good shape, furniture and supplies are appropriate to meet the needs of the program.
LPA reviewed five (5) client files and two (2) staff files. Files contain appropriate documentation.
Smoke detectors and carbon monoxide detectors are operational in the facility. Fire extinguishers and first aid kit are maintained and ready for emergency use.
At this time, the program appears to be in substantial compliance with the regulations. No deficiencies were cited at this visit.
