On July 9, 2026, at 10:45AM, Licensing Program Analyst (LPA) Edward Kim conducted an unannounced Collateral visit. LPA Kim met with Licensee Paul Chierichetti and explained the purpose of the visit. The purpose of this Collateral visit was in reference to a complaint with Complaint Control Number: 22-AS-20260428141306.
LPA interviewed resident #1 in private room due to complaint allegations that occurred at another licensed facility.
No deficiencies were cited during the inspection visit.
An exit interview was conducted, and a copy of this report was provided to Licensee Paul Chierichetti .
