Facility Evaluation Report

On November 15, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose the visit. Aisa Guiling was notified via telephone and later arrived to assist with the inspection.

On today's inspection, LPA observed four clients in care. LPA, accompanied by a staff, conducted a tour of the facility. LPA inspected areas such as the client bedrooms, bathrooms, kitchen, dining room, living room, and garage. LPA observed the facility to be clear of any obstructions and hazards. LPA observed that the facility went through a change of ownership application and is now operating under licensed facility, Solaire Home - 306006677 as of April 15, 2025.

The Department will process the facility closure with an effective date of April 15, 2025. The facility surrendered their license on today's visit.

Based on the observations made during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Aisa Guiling and a copy of the report was provided.
