Therefore, the cabinet lock was rendered faulty, leaving medications unsecured. Prior to LPA’s visit, HM Aguilar has addressed the issue with the medication cabinet lock with Administrator (AD) Oluwatosin Ajijola. AD Ajijola fixed the faulty lock but as of today’s visit the medication cabinet is unable to consistently lock properly making it accessible to clients.
Therefore, based on LPA Tea's observations and interview conducted, the allegation facility centrally stored medication is accessible to facility clients due to a faulty lock is determined to be SUBSTANTIATED, meaning the complaint allegation is valid and that a violation has occurred.

An exit interview was conducted with House Manager Stephanie Aguilar and Administrator Oluwatosin Ajijola over the phone and a copy of this report and appeal rights was provided to the facility.
