The inspector’s account

of May. Their new administrator (AD) is Mr. Oluwatosin Adeyinka Ajijola, who goes by the name of Joe. On the recently updated Personnel Report (LIC500) shows AD Ajijola listed as the facility administrator with a date employed of May 1, 2025. HM Aguilar explained that at the time in April there was no qualified administrator, the administrator for the facility had an expired administrator certificate. The licensee, Dr. Miatta Snetter, is a qualified administrator, that was spending time mainly at the other facility in La Habra.

It was alleged that staff did not meet training requirements. The investigation determined the following: LPA looked through the staff files and reviewed the staff training. 1 out of 9 staff files reviewed had current DSP training. Another staff out of 9 staff files reviewed showed different training in August 2024. Most of the staff had outdated training that was done in March 2024. As of now, since May 2025, staff have enrolled and completed their required DSP training.

Therefore, based on LPA Tea's observations, interviews conducted, and records reviewed the allegations that facility does not have a qualified administrator and staff did not meet training requirements are determined to be SUBSTANTIATED, meaning the complaint allegations are valid and that violations have occurred.

An exit interview was conducted with Administrator Oluwatosin Ajijola and a copy of this report and appeal rights was provided to the facility.
