On today's date at 1:40 PM Licensing Program Analyst (LPA) Michael Tea made an attempted unannounced visit to the facility to conduct an Annual Required inspection visit. LPA Tea rang the door bell and nobody answered. LPA observed no one at the facility. LPA Tea then called and left voicemail message to the (LE) Elizabeth Maldonado. LPA also texted LE Maldonado. LPA will be attempting another visit later this month.
This report will be reviewed and signed by facility when LPA has successfully complete contact and inspection.
