Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit for the purpose of conducting a health and safety check. LPA was greeted and allowed entrance into the facility by staff. Licensee (LE) Rona Lomeda and Administrator (AD) Rochel Malaca arrived shortly to assist with the visit. LPA explained the reason for the visit.

During the inspection, LPA and LE Lomeda and AD Malaca toured and inspected the facility. LPA observed no health and safety issues. LPA Tea requested LE Lomeda and AD Malaca for a valid lease agreement for the property by close of business Friday, Oct 10, 2025.

Based on the observations made during today’s inspection, no deficiencies are being cited at this time.

An exit interview was conducted with Licensee Rona Lomeda and Administrator Rochel Malaca and a copy of this report was provided at exit.
