Facility Evaluation Report
On July 22, 2026, Licensing Program Analysts (LPAs) Eboni Bentley and Taylor Simerly conducted a case management visit after observing a deficiency while conducting complaint investigation into control #22-AS-20260720153805 and #22-AS-20260714162815.
Per record review, Staff #1 (S1), Staff #2 (S2), and Staff #3 (S3) are not associated to the facility. Administrator Isabel Ybarra stated, all three staff began working at the facility full-time on May 15, 2026. During today's visit, LPAs observed all three staff working at the facility and providing care to clients.
A deficiency is being cited per Title 22 Division 6 of the California Code of Regulations.
An exit interview was conducted, and a copy of this report, LIC809-D, LIC811, and appeal rights were provided to Administrator Isabel Ybarra.
