LPA Haddadin also interviewed four staff members; four out of four denied the allegations. During staff file review, LPA Haddadin observed documentation indicating staff are required to attend monthly training and demonstrate understanding of Client Personal Rights and confidentiality requirements. LPA Haddadin also observed documentation that staff completed medication management training regarding how to dispense and verify medications for clients in care. Additionally, LPA Haddadin observed staff administering medications to three clients and noted staff verbally verified each client’s name and date of birth to match the medication label and informed the client of the medication prior to administration. LPA Haddadin observed that both the client and staff signed the medication administration record following administration. During the facility tour, LPA Haddadin observed sufficient hygiene supplies available to clients, with additional supplies stored in the garage.
It was also alleged that staff do not provide adequate food service, staff do not ensure food is adequately stored, and the facility is in disrepair. All four clients and four staff interviewed denied these allegations. During the walkthrough, LPA Haddadin observed all four clients eating lunch prepared at the facility. During record review, LPA Haddadin observed documentation reflecting staff completion of a California Food Handler course and a State Food Safety certificate. LPA Haddadin also reviewed the facility’s weekly menu, which reflected planned meals including lunch and dinner, with breakfast available upon request. LPA Haddadin observed a variety of snacks available, including smoothies, snack bars, and chips. During the tour, LPA Haddadin did not observe conditions indicating the facility was in disrepair. LPA Haddadin also reviewed a maintenance log documenting repairs completed by the facility.
Based on observations, interviews, and record review, there is not a preponderance of evidence to prove or disprove that violations occurred. Therefore, the allegations are determined to be unsubstantiated.
An exit interview was conducted, and a copy of this report was discussed with and provided to the facility representative.
