Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a case management visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit.
The Department received an Unusual Incident/Injury Report - LIC 624 on June 22, 2026 for an incident that occurred on June 6, 2026 with Resident 1 (R1). It was reported that R1 had a wrist fracture and went to urgent care and was then transferred to St Joseph Hospital.
Per interview with Director of Health Services Breanna Creekmore via telephone Breanna stated that R1 had a fall on the floor on June 06, 2026 and the facility had ordered mobile x-ray which is when R1 was diagnosed with wrist fracture. It was reported then R1's Doctor advised R1 to be taken to urgent care for a splint. It was reported then R1 went to urgent care on June 07, 2026 and then the ER. R1 was discharged on June 8, 2026 with a splint and was placed on 1:1 care due to fall precautions.
LPA Mendivil obtained copies of LIC 602 Physician's Report, Service Plan and care notes for R1.
Based on observations no deficiencies cited during today's visit. An exit interview was conducted and a copy of this report was provided.
