Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a case management visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit.
The Licensee requested a new fire clearance to add an additional bedroom attached to the facility sunroom. The fire clearance was conducted and granted on 05/07/2026. LPA Mendivil along with Licensee Miriam Salcedo toured the room. LPA Mendivil observed the additional bedroom had the required elements including bed, closet space, lighting and ac unit. LPA Mendivil observed ample space to accommodate client's items.
No deficiencies cited and exit interview was conducted. A copy of this report was provided.
