Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit in conjuction with complaint control 22-AS-20260116134545. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit.
During the complaint visit LPA Mendivil observed construction on 2 additional dwelling units (ADUs) in facility's backyard. Administrator Carlos Vargas stated he is unsure of what the plan is with the ADUs. LPA Mendivil did not receive any notice or information regarding construction.
Based on observations made deficiencies are cited per Title 22. An exit interview was conducted and a copy of this report was provided along with appeal rights.
