Facility Evaluation Report
Licensing Program Analyst (LPA), Lydia Martinez made an unannounced visit to the facility for the purpose of conducting the Required - 1 Year Annual Inspection. LPA Martinez met with the Director Mark Desmond.
The facility currently provides services to 50 students, but not all attend the Day Program every day or are present the whole day.
LPA observed the required Department posting on COVID-19 precautions at entrance of facility. There is a sign-in procedure in place and hand sanitizer for use. LPA observed that all staff were wearing face masks. The facility has an approved Mitigation Plan on file. LPA toured class rooms and activity rooms, all rooms where within regulations. Restrooms observed contained soap, toilet paper and paper towels. Restrooms had proper hand washing signs posted. Facility has operating smoke detectors. Facility has Fire Extinguisher which was charged. The facility was equipped with sufficient hand hygiene supplies, cleaning and disinfecting provisions. Personal Protective Equipment (PPE) supply is available. The facility monitors the students regularly for any COVID-19 symptoms/change of condition and documents. The facility does not provide meals or snacks for the students, only allows students to store their lunches in the facility refrigerators. Students are required to bring their own meals and snacks. During the visit LPA observed all toxins and other hazardous items were locked up and inaccessible to students in the janitorial closet and in locked cabinets in the kitchen area. Hot water temperature is tested and is within regulatory requirements. Files have current Needs and Services Plans (ISPs or IPPS), Admissions agreements, and Physician's Reports. First Aid and CPR training, and other documented training is up to date. Fire Drill was last conducted on 07/14/2021.
Based on today's observations made during today’s visit, the facility appears to be in substantial compliance with Title 22 Division 6 of the California Code of Regulations. This report was discussed with Mr. Desmond and a copy of this report was provided
