Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit in conjunction with complaint 22-AS-20250821141505. LPA was greeted and granted entry into the facility and met with Life Skills Coach Erin Albright. LPA explained the reason for the visit. Administrator (AD) Sean Estrella was notified by staff via telephone.
On today's visit LPA Ramirez toured the facility and interviewed clients.
An exit interview was conducted with facility representative and a copy of this report was provided at the time of exit.
