Facility Evaluation Report
On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced to conduct a case management health and safety visit. LPA was greeted and granted entry into the facility and explained the reason for the visit.
The Department received an incident report for Client 1 and Staff 1 for an incident that occurred on May 16th, 2026. LPA Mendivil obtained copies of pertinent documents such as Individual Program Plan, LIC 602, needs and services and staff training records.
LPA Mendivil toured the facility and observed 1 client present during today's visit. LPA did not observe any immediate health and safety concerns during today’s visit. No deficiencies cited at this time.
An exit interview was conducted and a copy of this report was provided.
