Facility Evaluation Report

During the investigation into complaint control number 22-AS-20250923151212 it was discovered the facility failed to report a fall and an incident involving another facility resident.

While investigation the complaint control number above, during document review it was discovered that Resident 1’s (R1) fall that resulted in hospitalization due to a head injury was not reported to the Regional Office. There was also another incident involving R1 and another resident on August 28, 2025, that was not reported to the regional office.

As a result of today’s Case Management visit, a deficiency will be cited for failing to report.

An exit interview was conducted and a copy of this report and appeal rights were provided.
